Male infertility: assessment and treatment in Rabat

When a couple has not conceived after about a year of regular unprotected intercourse, a male factor is involved in nearly half of cases. In Rabat, Dr Najib Abakka assesses the male partner and, depending on the cause, offers medical or surgical treatment or sperm retrieval for assisted reproduction.

Conditions we treat

  • Abnormal semen analysis (sperm count, motility, morphology)
  • Azoospermia (no sperm in the semen)
  • Varicocele
  • Undescended testicle
  • Hormonal disorders

When should you seek help for male infertility?

  • No pregnancy after 12 months of regular intercourse
  • After 6 months if the female partner is over 35
  • An abnormal semen analysis
  • History of undescended testicle, surgery or genital infection
  • Varicocele or testicular pain
  • History of chemotherapy or radiotherapy

The male assessment

The assessment combines a medical history, a testicular examination and one or more semen analyses. It is completed when needed by hormone tests, a scrotal ultrasound and, in some cases, genetic testing. It is carried out in coordination with the couple's gynecologist and fertility center.

Treatments offered, depending on the cause

  • Addressing contributing factors (smoking, weight, heat, medication)
  • Hormone therapy when indicated
  • Microsurgical varicocele repair under high magnification
  • Surgical sperm retrieval
  • Micro-TESE under high magnification for azoospermia
  • Surgery for an undescended testicle

Frequently asked questions

What is micro-TESE?

Micro-TESE (microdissection testicular sperm extraction) is a procedure that looks for sperm directly in the testicle, under high magnification, in men with azoospermia. Any sperm found is used for in vitro fertilization with ICSI.

Should a varicocele be operated on?

Not always. Surgery is discussed when the varicocele can be felt and is associated with an abnormal semen analysis in a man trying to conceive, or when it is painful.

Is a single semen analysis enough for a diagnosis?

Not always: results vary from one test to another and are often repeated. They are interpreted alongside the clinical examination and, when needed, hormone tests and ultrasound.
